How we can help

 

In broad terms the development of the Bridging Service will lead to increased CHCP staff awareness of employability services enabling workers to help clients / patients move towards and into work, and employability staff awareness of barriers to employment. 

The Service will also work directly with a number of clients / patients helping them access relevant employability services and subsequently moving into work, increasing their overall wellbeing.

Key outcomes will be:

    • Enhanced trust and understanding between agencies and between potential clients and support services
  • Improved information sharing across services and agencies, including participation in joint briefings and training
  • Increased referrals across agencies, with an emphasis on the role of the CHCP
  • Development and implementation of a referral tool, for incorporation within CHCP services in particular
  • Highlighting areas for service improvement, including tackling obstacles that remain
    • Informing further debate on Glasgow’s approach to employability issues.

It is envisaged some of these staff will be dedicated to this service with a dual remit of assessment/client management and specialist input.

The team will provide a multi disciplinary approach for clients and will work with health and social care staff to raise awareness of employability services as well as providing an assessment and referral service-to-service users.

The service will offer a single point of contact for initial assessment, guidance and support provided by members of the core team. Clients may then be referred on to further employability and related services.
